Milstein obtained his earliest opportunity in the music industry during 1983, after Chaka Khan included a composition he had co-authored on her album I Feel for You. While then living in Boston and studying music, he traveled to New York upon receiving the news and participated in the sessions with producer Arif Mardin. The next year he established residence on the west coast. In Los Angeles he teamed with keyboardist Jeff Lorber to form a band whose live engagements covered Japan and the United States and whose studio work followed soon afterward. Their partnership encompassed arranging, songwriting, and production for artists that included Manhattan Transfer, Juan Gabriel, and Michael Franks, among others. Milstein’s subsequent projects involved several prominent vocalists. In 1991 he served as bassist for Bette Midler on a Christmas recording. Three years later he joined Diana Ross for the tour that traversed South America and Europe in celebration of her thirtieth anniversary as a former Supreme. After the dates concluded, Milstein visited his mother’s relatives in Uruguay and Argentina, where indigenous musical elements registered an immediate and forceful impact. Those Latin influences later surfaced on his album Dancing in the Rain.
