In 1991 Chicago bandleader Tim Cain launched performances with a four-piece backing group specifically to examine gay themes. An advertisement aimed at queer musicians drew only two replies, one from rhythm guitarist Mike Ferro. (Ferro and his partner also claimed bodybuilding medals at the 1994 Gay Games.) Cain reconnected with former high-school classmate Cie Fletcher, who assumed lead guitar and shared production responsibilities. Boys' Entrance made its first appearance at Chicago's Gay Pride rally in June 1994 before an audience of roughly 1,500. Two EPs and a full album appeared soon after, although ongoing personal tensions within the band eventually resulted in Fletcher's dismissal.
