In the peak period of hair metal, Extreme appeared well-matched to the glam aesthetic, complete with voluminous hairstyles, memorable riffs, the commanding vocals of Gary Cherone, and Nuno Bettencourt's guitar technique that could stand alongside Eddie Van Halen. On Extreme II: Pornograffitti the Boston-based outfit showed a clear desire to move past stereotypes by exploring heavier metal textures while also embracing funk, pop, and the gentle Everly Brothers-inspired harmonies of "More Than Words." That track delivered Extreme their first number one single in 1991, a feat nearly matched later the same year by the driving "Hole Hearted," yet external shifts soon intervened: when III Sides to Every Story arrived in 1992, mainstream rock had already turned toward flannel, after which Cherone departed to succeed David Lee Roth in Van Halen. Bettencourt disbanded Extreme soon afterward, although the musicians reconvened in 2004 and issued Saudades de Rock in 2008. Despite taking fifteen years to complete their next record, Six, which surfaced in 2023, the group never truly disbanded, maintaining a steady schedule of live performances throughout the intervening period.

The Massachusetts-based ensemble came together in 1985; guitarist Nuno Bettencourt had previously performed locally with bassist Pat Badger, while singer Gary Cherone led his own band alongside drummer Paul Geary. The separate units eventually combined forces, and Cherone and Bettencourt established a songwriting alliance that would soon elevate Extreme to the top of the Billboard charts.

Pairing Bettencourt's technical guitar skill with a funk-infused rhythmic foundation, the band released its self-titled debut album in 1989. The set reached number 80, a respectable start for an emerging act, and the track "Play with Me" provided the music for the chase sequence in Bill & Ted's Excellent Adventure. Extreme began to gain recognition within heavy metal circles, yet it was the arrival of Pornograffitti, especially the acoustic Everly Brothers-style ballad "More Than Words," that connected the group with a broader audience. "More Than Words" climbed to number one, and its follow-up, the acoustic pop-rocker "Hole Hearted," peaked at number four. Extreme had become one of pop-metal's leading acts, but the rising tide of grunge together with the band's evolving preferences rendered their commercial dominance brief.

The third album, III Sides to Every Story, represented an ambitious undertaking whose initial sales, though solid, ultimately fell short of Pornograffitti's totals. Geary exited the group shortly thereafter to pursue a career in the music industry, and a reconfigured lineup featuring Mike Mangini, formerly of Annihilator, on percussion returned in 1995 with Waiting for the Punchline. That release likewise struggled commercially. Consequently, Extreme announced their breakup the following year. Bettencourt pursued a solo path and delivered the Schizophonic album in 1997 before issuing two further records with Mourning Widows. Cherone, meanwhile, spent a short time as Van Halen's vocalist; his sole album with the group, 1998's Van Halen III, drew widespread criticism from reviewers and fans, leading to disappointing sales and Cherone's rapid departure.

A thirteen-track best-of collection titled An Accidental Collision of Atoms preserved Extreme's catalog in 2000, and the band quietly regrouped in 2004 for a limited run of concerts. Those reunion performances extended into 2008, when Saudades de Rock became the first collection of new songs in thirteen years. A lengthy tour followed, and while the musicians continued to develop fresh material and play occasional shows, Bettencourt's touring commitments with Rihanna and Cherone's project Hurtsmile kept Extreme on the periphery. In 2016 the band issued Pornograffitti Live 25: Metal Meltdown, documenting their 2015 concert at Las Vegas' Hard Rock Casino in audio and video form.

Extreme conducted multiple international tours through the final years of the 2010s while steadily developing their sixth album. The resulting Six finally appeared in June 2023, fifteen years after Saudades de Rock.
