Keith Thornton, recognized as Kool Keith and the singular creative force within Bronx hip-hop collective Ultramagnetic MC's, launched an array of solo ventures that propelled him far beyond conventional bounds, none more impactful than the pioneering Dr. Octagon project. Under this alias he embodied a deranged alien gynecologist-surgeon whose rhymes retained the same unbound associative density and multi-layered imagery once displayed with his New York crew, along with pointed asides aimed at those who "water down the sound that comes from the ghetto." The character's 1996 debut album surfaced as an unexpected cult success, gaining wider exposure through a 1997 major-label reissue before solidifying its status as one of underground hip-hop's most enduring and influential recordings. Although Keith later attempted to retire the persona, its grip on listeners persisted, prompting two unofficial sequels before the original album's core participants reconvened for an authentic continuation in 2018.

The project's first single, "Earth People," appeared in late 1995 on San Francisco imprint Bulk Recordings and quickly circulated throughout the hip-hop underground, mirroring the rapid spread of the self-titled full-length issued the next year. Internationally acclaimed turntablist DJ Q-Bert of Invisibl Skratch Piklz supplied scratches while the Automator handled production, resulting in a left-field blend of sound collage, aggressive scratching, and surreal, impressionistic flows that reached audiences ranging from dedicated hip-hop enthusiasts to disillusioned rock writers. Though juvenile fixations on anatomy and bodily functions often surfaced, Keith's intricate network of references and shifting allusions frequently revealed remarkable depth. The album reached U.K. abstract-hip-hop label Mo'Wax, home to DJ Shadow, for European distribution in mid-1996, after which the imprint issued a DJ-oriented instrumental edition titled The Instrumentalyst.

That broad appeal secured Keith a 1997 deal with DreamWorks, which reissued Dr. Octagon as Dr. Octagonecologyst with additional tracks. Early the following year, however, his alternate character Dr. Dooom dispatched Dr. Octagon on the opening cut of First Come, First Served, released via Thornton's own Funky Ass imprint. Kool Keith subsequently joined Ruffhouse/MCA for the 1999 release Black Elvis/Lost in Space. Further Kool Keith albums arrived in 2000 with Matthew and in 2001 with Spankmaster, while the 2002 collaboration Gene appeared under the KHM banner alongside H-Bomb and Marc Live. His next undertaking formed the four-member group Thee Undatakerz, in which Keith assumed the identity of Reverend Tom; Kool Keith Presents Thee Undatakerz reached stores in May 2004. That August he issued Diesel Truckers with longtime collaborator KutMasta Kurt.

Even amid the 2004 deluge of material, several eccentric releases surfaced, including the mixtape The Official Space Tape assembled from prior recordings, the limited-edition R&B-oriented Personal Album pressed in only 500 copies at a premium price, and the unauthorized Dr. Octagon, Pt. 2 on Real Talk, which courts swiftly removed. The subsequent year proved quieter, bringing the Global Enlightenment, Pt. 1 DVD early on and Lost Masters, Vol. 2 during the summer. Nogatco Rd. introduced the reversed-character Mr. Nogatco in 2006 through a UFO-themed collaboration with producer Iz-Real. Shortly afterward came the two-disc Collabs Tape anthology and The Return of Dr. Octagon, the latter produced largely without Keith's participation.

In early 2007 Keith rejoined Ultramagnetic MC's for the reunion album Best Kept Secret. The concept-driven Tashan Dorrsett followed in 2009, with its remix counterpart The Legend of Tashan Dorrsett arriving in 2011. Love and Danger, issued in 2012, contained tracks such as "Goodbye Rap" that suggested retirement, yet Keith remained active the next year with a guest spot on the Yeah Yeah Yeahs' Mosquito, credited once more as Dr. Octagon. In 2015 he partnered with L'Orange for the co-billed Time? Astonishing! and with Ray West for A Couple of Slices, issued both as a standard LP and a limited pizza-box deluxe package containing an extra 7" and cassette. Two further 2016 projects, The Preacher and Feature Magnetic (the latter featuring MF Doom, Godfather Don, Sadat X, and additional guests), preceded the 2018 revival Moosebumps: An Exploration into Modern Day Horripilation. This release marked a genuine sequel by reuniting Keith with the Automator and DJ Q-Bert for the first time since the original album.
